Your first appointment is an Initial Assessment in which you and your psychologist discuss your needs and determine a plan of action. An Initial Assessment usually takes about an hour; however, you should arrive at least a half hour before your scheduled appointment time so that you can read and fill out the necessary forms.

Forms:
Before your Initial Assessment, you will be asked to: 1) read our Privacy Notice, 2) read and sign our Client Services Agreement, and 3) complete an Initial Assessment form. You can save time by printing out the forms, completing them prior to your first visit, and bringing them with you to your appointment.

You will need a password from Dr. Ochester in order to open the printer-friendly versions of these forms. You can obtain the password from Dr. Ochester when you call to make your first appointment. You can download the latest version of Adobe Acrobat Reader (PDF) free from their website.
